The following information includes a summary of benefits available at the Illinois State Water Survey. For more complete, official information, please refer to the "Common Policy and Information Manual" and the Illinois State Water Survey's "Internal Policy and Procedures Manual." All specific questions or requests for additional information should be directed to the Human Resources Office. HolidaysTwelve scheduled holidays plus General Election are observed by the Survey: New Year's Day, Martin Luther King's Birthday, Lincoln's Birthday, Washington's Birthday, Memorial Day, Independence Day, Labor Day, Columbus Day, Veteran's Day, Thanksgiving (Thursday and Friday), and Christmas plus General Election. Vacation LeaveFull-time staff members earn 2 work days of vacation per month (24 days/year). Effective March 1, 2003, a maximum of five (5) days of annual leave may be carried over to the next calendar year. Part-time staff earn on the same basis, prorated in proportion to their percent time. Vacation is posted at the beginning of the calendar year, except for staff during the initial probationary review period which is earned at the rate of two days per month. Vacation leave may be taken before it is earned, but a staff member separating from service is liable for any unearned leave which is taken and much reimburse the Survey. Vacation leave may be taken in increments of 15 minutes and should be scheduled in advance with the supervisor. Upon resignation or retirement, an employee is compensated for unused vacation. Personal LeaveFull-time staff earn 1.875 hours/month (3 days/year) of personal leave; part-time staff earn on the same basis, prorated in proportion to their percent of time. Staff members who do not use any sick leave in a calendar year are awarded an additional personal day on the following January to be used within that calander year. Personal leave does not accrue from one calendar year to another. Personal leave may be taken in 15-minute increments and may be taken before it is earned. Sick LeaveFull-time staff earn 1 work day of sick leave per month (12 days/year) on a cumulative basis. There is no maximum number of sick days that may be accumulated. Part-time staff earn on the same basis, prorated in proportion to their percent time. Sick Leave is Noncompensable but can be used for service credit in the retirement system. SURS Retirement SystemThe State Universities Retirement System of Illinois (SURS) provides retirement, disability, death, and survivors' benefits to all eligible SURS participants and annuitants. You are automatically a SURS participant if you are employed in a position that requires you to work for at least one continuous academic term or 4 months whichever is less. The employee contributes 8% of gross earnings. (No Social Security taxes will be withheld from your earnings, however, contributions for Medicare will be withheld.) Two years of service qualifies an employee for disability benefits. Survivor benefits are payable to a surviving spouse, dependent parents, or unmarried children under 18 if 1.5 years of service credit has been established. An employee becomes vested after 5 years of service credit. There are three plans to choose from under the State Universities Retirement System - Traditional, Portable, or Self Managed. New employees have 6 months to choose their Retirement Plan. Their choice is a one-time lifetime choice. MedicareMedicare coverage for new employees is deducted at a rate of 1.45% of gross earnings. Health InsuranceEmployee's health insurance is provioded by the State. The employee makes a small monthly contribution based on annual salary and can purchase health insurance coverage for eligible dependents. In addition, several health maintenance organization plans are available. (An HMO is an organization operated within local medical facilities which agree to deliver a comprehensive set of health care benefits to persons living within a defined area on a prepaid, fixed payment basis. For a fixed monthly payment for dependents, the HMO will provide virtually all health care needs with elimination of any billing for services or processing of claim forms.) Quality Care Health PlanThis plan offers a comprehensive range of benefits, as well a some features not available through HMOs. Under the Quality Care Health Plan, the employee is free to choose any provider and to change providers at any time. Health Maintenance OrganizationsHMOs are managed care plans which provide comprehensive benefits on a prepaid basis. There are differences in the premiums and copayments among the various plans. HMO members must choose a primary care physician from those participating in the HMO provider network. Dental InsuranceStaff who are eligible to participate in the health program are provided with dental insurance if they choose. Part-time staff will receive coverage based upon their percent time appointment. Quality Care Dental PlanThe employee may go to any dentist for treatment and receive a broad range of coverage. Participants are required to satisfy an annual plan deductible for non-preventative services. Preventative services are not subject to this deductible. This plan reimburses a predetermined or "scheduled amount" for each covered service. The employee is responsible for any amount over the scheduled benefit. Vision Care PlanVision benefits are available to all members and dependents who are enrolled with health coverage at no extra cost. The plan is designed to encourage maintaining vision through regular eye examinations and to aid with vision care expenses when glasses or contact lenses are required. Life InsuranceThere are two types of coverage available: basic coverage and optional coverage. Basic Coverage (state paid)Term life insurance is provided through the State Employees Group Insurance Program. This benefit is provided automatically by the State at no cost to full-time employees. Each eligible, active, nonretired employee is insured for an amount equal to his/her salary. Optional Coverage (member paid)Employees may purchase optional insurance at low group rates. All premiums for any optional insurance are at the member's expense. Optional life insurance includes life insurance for member equal to one, two, three, or four times the State-provided amount, Accidental Death and Dismemberment coverage for member on the basic amount only or the optional plus basic, spouse term life insurance ($5,000) and child term life insurance ($5,000). University of Illinois Employees Credit UnionSurvey employees are eligible to become members of the Credit Union. Payroll deductions can be arranged for loan payments and/or savings. Deferred Compensation or Tax Sheltered Retirement PlanThe Deferred Compensation Plan (available through State-funded payrolls) and the Tax Sheltered Retirement Plan (available through University of Illinois payrolls) provide an optional benefit to employees whereby a designated amount of the employee's salary is withheld each pay period in order to defer the tax liability on that portion of salary. This is an optional, long-term savings program using pre-tax dollars through payroll deduction. Currently, there are seven investment choices. Employee Assistance ProgramDesigned to assist employees and their families with non-work-related problems. Savings BondsSavings bonds purchased through payroll deductions provide an easy, convenient method of saving regularly that offers tax advantages and near-market interest on small sums of money. Flexible Spending AccountsTwo types are available: Dependent Care Assistance Program (DCAP) and Medical Care Assistance Program (MCAP). Employees are able to pay dependent care expenses and certain medical expenses with tax-free dollars while reducing annual taxes. Probationary ReviewNew staff appointments are subject to probationary review. The employment of any individual not performing satisfactorily may be terminated at any time within the probationary period. The probationary period is 12 months for all employees. Work ScheduleFull-time members of the professional staff are to be available for duty at all times except when on leave. Normal work hours are 8 am-noon and 1 pm to 4:30 pm, Monday through Friday.
